Handle compressed headers.  Fix inappropriate use of bytestream_flush().
Code cleanup.  Added getcaps and _link functions for src pads.  Extract
and set the size,rate,channels correctly.  Fix some of the caps to
agree with avidemux and/or ffmpeg.
