ext/libvisual/visual.c: Handle DISCONT.

Original commit message from CVS:
* ext/libvisual/visual.c: (gst_visual_reset),
(gst_visual_sink_setcaps), (gst_visual_sink_event),
(gst_visual_src_event), (get_buffer), (gst_visual_chain):
Handle DISCONT.
Use running time before doing QoS.
Handle mono too.
