matroskademux: Expand the tag mapping.

* Also expose unknown tags as key=value pairs.
* Arrange tag map in the same order tags are listed in Matroska spec, leaving
unmapped tags as comments.
* More specific TODOs.
* Remove duplicate DATE define.
